To Order a Birth Certificate by Mail:
Print and complete the certificate request form. Send a letter of request, the completed form $25 per copy, and self addressed stamped envelope to the Highland County Health Department, 1487 North High St., Suite 400, Hillsboro, Ohio 45133. We accept money orders and local checks only.
